Immunization Form

According to New York State public health laws there are two immunization requirements that students must meet before registering for classes. New York State public health law 2165 requires that all students born after January 1, 1957 must document immunity to Measles, Mumps and Rubella. New York State public health law 2167 states that colleges must inform their students about meningococcal meningitis and the vaccine that protects them against most strains of the disease that can occur on college campuses. Students must then make an informed decision as to receive the Meningitis vaccine if they have not already or officially document that they decline to receive it.

Please submit the attached immunization form to the Office of the Registrar upon completion.
